Mental Health Practitioners are within a variety of surgeries offering mental health assessment, formulation and clinical interventions based on Nice guidance, brief solution focussed interventions and trauma informed care.

The Mental Health Practitioner will provide a clinic whereby patients can be booked in to be seen by the Mental Health Practitioner who will either provide mental health support tailored to the individuals need and community connections or utilise the Living Well service to strengthen the care and support the person requires.

The Mental Health Practitioner will offer a response to people with common mental health needs through and agreed number of weekly slots for GP appointments and carry a small caseload. As a Mental Health Practitioner, interface and relationships with our partners including the ICO and neighbourhood leadership teams is essential.

Mental Health Practitioners in Manchester will offer a clinic to assess the mental health and social care needs of individual at the request of the GP. As part of the wider Primary Care team, you will promote and implement early intervention, avoiding unnecessary medication, referrals and hospital admissions, and support service users to self-care, whenever possible.

You will be a conduit between Primary care, Living Well, secondary care and other agencies ensuring warm introductions and smooth seamless journeys for service users.

The Mental Health Practitioner role will mainly work with people who don’t meet the threshold for Community Mental Health Teams, those who are not responding to primary care mental health interventions or are not appropriate/not ready for IAPT, thus widening access to mental health support across services.
