We are looking for a passionate individual to help us make a difference and join us as a Health & Wellbeing Coach delivering on both our Healthier Futures and NHS Diabetes Prevention Programme (NDPP)

Healthier Futures offers dedicated support to the people of East Riding of Yorkshire to empower, educate and equip them to live healthier lives and enjoy a healthier future. The service consists of three distinct programmes; **Healthier Lifestyles** (adult weight management), **Quit Smoking**, and **Healthier Together** (family weight management). NDPP supports service users through a 9-month journey via 1:1 and group sessions.

This is a dual role working 50% on delivering our NSH Diabetes Prevention Programme in Humber & North East Yorkshire and 50% in our Integrated Lifestyle services in East Riding.

**Every day you will...**
- Deliver weight management and smoking cessation programmes to service users aged 12 and above through one to one and group-based models, either face to face or remotely
- Make onward referrals and signpost service users to external local and national specialist support agencies
- Attend community events and promote the service in a professional and knowledgeable manner
- Work closely with service users throughout the programme delivering a first class service and knowledge
- Provide guidance and support whilst taking service users on their journey throughout the programme
- Maintain data monitoring records of all service users and ensure that this is handled with the greatest care
- Consistently check and monitor the progress of the service users

**To thrive in this role you must have...**
- A BSc in a Sport & Exercise Science OR Nutrition related degree OR a REPS level 3 in Personal Training
- Previous experience in nutrition with a passion for health improvement
- Previous experience in 1:1 or group delivery
- A full drivers license and access to a vehicle
