**Role**: Triage Coordinator / Intermediate Care Multi Therapy Assistant
Reading Intermediate Care Team, Berkshire West
**About the Job**:
- To be an integral member of the triage team - to assist with the management of referrals, to supporting the integrated triage process and Sitrep flow calls
- To ensure that patients taken on through the service are managed in accordance with a clear care plan identifying their needs and ensuring appropriate actions are taken.
- To be able to carry out first/make safe visits to improve patient flow and safe discharge of clients in the community.
- To discuss patients, progress while on the service and review with the multi-disciplinary team as appropriate.
- To assist the patient to recognise when their personal agreed goals have been achieved.
- You will be expected to liaise effectively and professionally with all levels of staff, external organisations, and the public, dealing with confidential information in an appropriate manner.
- You will provide quality reporting service, statistics and audits in line with Government and Trust requirements.
- You will have a broad knowledge base, good time management skills and be computer literate and be able to demonstrate relevant experience for this role.

**The “must haves” for you to be considered for this role**:
1. An NVQ level 3 or higher qualification or an equivalent.
2. Minimum experience of 3 years in the community services supporting patients in their homes/place of residence.
3. Evidence of Statutory & Mandatory Courses
4. Car driver with ability and readiness to travel to multiple sites
5. Ability to use MS Excel, Teams, Word and Power point systems effectively
This role is in scope for the Vaccination as a Condition of Deployment (VCOD) legislation which is currently under review. We encourage and support vaccine uptake as this remains the best line of defence against COVID-19.
