**Key Responsibilities**:
**Personalized Care**:Provide personalized assistance with personal hygiene tasks, such as bathing, grooming, and dressing.
Administer medication according to established care plans.

**Mobility and Exercise Support**:Assist clients with mobility, transfers, and prescribed physical exercises.
Implement strategies to enhance clients' mobility and independence.

**Meal Assistance**:Prepare and serve nutritious meals, considering dietary preferences and restrictions.
Support clients with feeding, as necessary.

**Emotional and Social Support**:Foster companionship by engaging in meaningful interactions.
Offer emotional support to alleviate feelings of loneliness and isolation.

**Household Management**:Assist with light housekeeping tasks, ensuring a clean and safe living environment.
Support clients in organizing their living space.

**Health Monitoring**:Monitor and document vital signs and health indicators.
Report any changes in clients' health conditions to the healthcare team.

**Transportation Assistance**:Provide support with transportation for medical appointments, grocery shopping, and other essential errands.

**Documentation and Reporting**:Maintain accurate and up-to-date client records using Gap Kare's documentation system.
Contribute to care planning meetings by providing relevant reports.

**Adherence to Policies**:Strictly adhere to Gap Kare's policies and procedures.
Ensure compliance with relevant legislation and regulations governing care provision.

**Qualifications and Requirements**:

- Previous experience in a care role, preferably in a home care setting.
- Relevant certification or qualification in health and social care.
- Familiarity with health and safety practices.
- Excellent communication skills, both verbal and written.
- Ability to work independently and collaboratively within a team.
- Demonstrated compassion, empathy, and a genuine commitment to enhancing clients' quality of life.

**Working Conditions**:

- The role may involve flexible hours, including evenings and weekends.
- Travel to clients' homes is required; therefore, a valid driver's license and access to transportation are essential.
