We are looking for an enthusiastic and committed individual with exceptional communication skills to join our team as a Physiotherapy Assistant Practitioner within the Nottingham Stroke Service. This service provides specialist assessment and rehabilitation to adults across the East Midlands. This role is based primarily within the hyper-acute stroke service at QMC but should the service demand this may expand into other areas in the stroke pathway.

The Physiotherapy Assistant Practitioner will work closely with the multidisciplinary team to provide timely assessment and discharge planning to stroke patients on the hyper-acute ward.. The post holder will deliver stroke assessments and pre-defined rehabilitation intervention without direct supervision from a clinician within the ward setting. There is an expectation to carry your own workload and be integral to the fast pace running of the ward environment.

You will need excellent time management skills. You must have previous experience of working in a rehabilitation environment as well as within a hospital setting. Experience of stroke is desirable. An understanding of the physicality and handling required within stroke rehabilitation is an essential part of the post.

You will actively participate in service improvement, training, supervision and yearly appraisal to continually develop your skills, enabling safe and effective patient care.
You will work alongside the multi-disciplinary team in the delivery of rehabilitation for patients, as well as delegated admin duties.

The Therapy Assistant Practitioner (TAP) provides appropriate assessments, treatment and discharge planning for patients under the supervision of a senior therapist.

The post holder will be required to work closely with the multidisciplinary team to develop and implement rehabilitation programmes, as well as facilitate discharges with the support of senior staff.

To use effective communication skills to gain co-operation with treatment where there may be significant barriers to understanding or ability to co-operate with the treatment.

To discuss caseload management with other clinicians and provide advice and support on a day to day basis to other support workers.

To refer onto appropriate hospital and community services.
