Family Discharge Assistant

A new opportunity has arisen for aFamily Discharge Assistantto support patients and relatives in the process of discharging from the acute hospital. This is an exciting position and would suit someone who is keen to work in an integrated way and to support their continued professional development.
The role will require you to work weekends on a rota basis.
The role will require you to work across all ward areas including those that are designated as positive, asymptomatic and negative COVID wards.
To assist patient and families through the discharge process and provide support to the ward staff and the hospital MDT on the wards. The post holder will be required to assist and ensure timely and safe discharge from the acute trust.
To ensure that the patient flow team are updated regularly on the progress of potential and definite discharges identified on the ward, progress and assessing the delays in the patient journey to ensure Red days are turned to Green days, embedding SAFER principles, whilst ensuring MDT reviews are accurately updated. Proactively identify and support patient to transfer to the discharge lounge, ensuring appropriate transport arrangement have been arranged for a timely discharge. Ensuring the Discharge tracking board is updated in the agreed time frame. The post holder will also support and assist the discharge coordinators in dedicated to the ward area.
Ensure clear communication is provided to patients and their relatives at all points
Ensure that the ward team and professionals from other agencies and services have available appropriate patient information to facilitate assessment and discharge planning alongside the MDT.

Key results from the job holder
- Communicate with patients, relatives and other staff members regarding the proposed discharge arrangements as agreed by the acute discharge team.
- Ensure EDS are completed on admissions by all members of the MDT and clear document on the discharge tracking board.
- Monitor, and analyse the progress of referrals/assessments that are required to assist with the discharge process to support the IDT.
- Support ward teams by facilitating the process of requesting TTO’s / Transport/ Discharge Summary including progress chasing diagnostics, medical reviews etc. to assist with a timely discharge and avoidance of any delays.
- Ensure GP’s receive discharges letters in real time.
- Liaise with both the nurse in charge of the ward and/ or Bed Manager to facilitate and plan for vacant bed availability as part of the new bed management programme.
- Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals
- Assist to maintain safety, conforming to health, safety and security legislation, policies, procedures and guidelines.
- Maintain and develop own competence
- Ensure all medical and clinical ward rounds are accompanied.
- Support and escalate hospital reviews.
- Monitor and provide reports of discharges per day prior to 10.30am.
Planning and Organisational Duties
The post holder will participate in, and influence, care delivery by:
- All relevant documentation to assist with the discharge process are complete, ideally from the time of admission
- Communication with a range of clinical and non-clinical staff concerning the discharge process for patients
- Identify potential care needs for discharge at the time of admission and commence early planning for the patient’s potential needs on discharge, including referrals to relevant services.
- All relevant paperwork is completed by the appropriate persons to ensure an efficient and timely discharge.
- All relevant tests have been requested and are expedited where required to assist with the discharge process.
- All referrals or assessments that have been requested including the repatriation of patients have been arranged by liaising with other care establishments via the bed management team.
- Attending the ward board and/or ward rounds
- Aware of expected admissions, both elective and emergency to the ward environment
- Any issues are highlighted to the appropriate Senior Manager that may affect the discharge process, for example a lack of senior doctors, therapy services and delays in test results/referrals.
