The post holder will be a clinical expert exercising high levels of judgement, discretion and decision making in clinical care. S/he will provide clinical leadership for the Community Response Services in Central Manchester locality and specific support with respect to Frailty across all Manchester localities and Trafford.

The post holder will be responsible for the overall clinical leadership, governance and pathway development of MCR services, working closely with the Locality Lead Nurse.

They will also work with system partners and contribute locally and strategically to the redesign of community urgent care.

Have a highly visible clinical profile in the locality, providing both advisory and clinical input into patient care, spending 50% of time in clinical practice managing their own case load and 50% of time supporting the ACPs within the locality alongside the citywide portfolio of frailty governance and senior oversight.

Work collaboratively with integrated teams and multidisciplinary teams to optimise clearly defined patient outcomes.

Work collaboratively with other Clinical / Medical Leads across the city to give peer support and support the MCR strategic objectives.

Work towards shaping and delivering a new multidisciplinary model of care for patients living at home with frailty, working with national best practice guidance and networking to develop effective services, linking with acute partners and being a driver and advisor in the development of integrated pathways that put the patients at the centre of care delivery.

Provide professional leadership and support to MCR services

Contribute to senior management team working, supporting and deputising for the MCR lead to ensure continued service delivery in their absence.

Contribute to the LCO On call rota
