An opportunity has arisen for an experienced qualified health professional (Nursing or Allied Health professional) to join the Transfer of Care Team at Blackpool Teaching Hospitals.

The role oversees multiple teams within the Transfer of Care Hub comprising of both Health and Social Care colleagues all working towards safe and timely discharge arrangements for our patients.

This is an exciting and high pressure role where delivering service improvements is essential.

The postholder will have expertise in discharge planning for acute patient pathways and have full knowledge of legislative and quality requirements in this field.

They will provide professional and managerial supervision/support to the team leaders they line manage and ensure service provision is maintained to a safe and high quality standard.

The post holder will be responsible for 'quality' management within the Team including CQC evidence compliance and working with Partner organisations such as Adult Social Care to improve outcomes and experience for patients being discharged from Blackpool Hospitals.

The Transfer of Care team was set up in October 2021 and comprises a full service support for all patients on National discharge pathways 1, 2 and 3. There is a dedicated 'Homefirst Team', Discharge Facilitation Team, Care Home Liaison and Complex Case Nurse Teams. Plus full administrative support and Business Intelligence.

These Teams are co-located with our Partner Social Services within the Acute site giving the trust a uniquely integrated Transfer of Care team.

Main duties include
- Day to day operational running of the services under portfolio.
- Quality lead for the Transfer of Care team
- Professional supervision and support for direct reports.
- Deputising for Clinical Lead for Discharge Services as required.
- Identifying areas within service that require improvement and working collaboratively to deliver redesigned services to meet that need.
- Ensuring self and all reports work within the National and Local requirements with policy and legislative frameworks
