**The team**

The Provider Service supports people with disabilities (learning disabilities, physical disabilities, or mental health conditions) to be influential in their local communities through citizenship, employment, enterprise, and volunteering opportunities. With a strong focus on the abilities of the individual and not their disabilities, we recognise the importance of supporting individuals to obtain their maximum potential and achieve the same life outcomes as those of people without disabilities.

Provider services are also key to supporting older people to remain in their own homes through facilitating seamless hospital discharges, preventing unnecessary admissions to hospital and residential care, by working with individuals to regain or maintain their independence with enablement and reablement approaches.

**About the roles.**

The Shared Lives Support Worker is the first point of contact for the Shared Lives Carers, for advice, guidance, and direct support. They are instrumental in supporting through induction, monitoring and supervision, as well as achievement and development processes. The Shared Lives Support Worker ensures that the care and support provided is appropriate to meet the identified needs of the individuals accessing the arrangement and is provided in accordance with the Care Quality Commission Regulations, the service policies and procedures, health and safety legislation, care and support plans.

The Shared Lives Support Worker is expected to Support the Shared Lives Carers to acquire and maintain the skills and knowledge to fulfil their role. The worker supports the Shared Lives Carers to have a positive relationship with those they support, which is respectful of individual right to privacy, choice, dignity, enabling the person to maintain and develop their independence and to be a valued member of their community.

It is essential that you have experience of supporting vulnerable adults, and of working both independently and as a part of a team member. It is important that you can demonstrate effective written, verbal and electronic communication skills and that you have achieved a minimum qualification of a National Diploma Level 2 in Health and Social Care or equivalent. You must also demonstrate knowledge of current legislation relating to people with disabilities i.e., Care Act 2014 and Equalities Act 2010, Safeguarding Adults, Mental Capacity Act, Care Quality Commission regulations and also have an understanding of the key issues relating to supporting a person with a disability along with knowledge of the Shared Lives Scheme and service.
