An exciting opportunity has arisen to join the Women and Children division at Lancashire teaching Hospitals Trust as an ASD Pathway Navigator.

We are seeking to recruit a highly motivated, confident and dynamic individual who will be a central point of contact for patients currently undergoing an Autism Spectrum Disorder Assessment; helping to ensure the individual needs of the patient are supported, whilst facilitating a seamless, coordinated pathway delivery, such as through escalation of requirements to relevant colleagues and supporting behaviour needs through signposting to local services.

You will work closely with the wider community paediatrics team (clinical and administrative), as well as other key health and social care providers, to support patients who are undergoing an ASD assessment. The ASD navigator will be involved from the beginning of the pathway up to the point of diagnosis (whether ASD or not). They will maintain effective communication both with and on behalf of the patient signposting to other resources and information as defined by patient need and liaising with staff to escalate any concerns as necessary. It is envisaged that the pathway navigator may develop this role and work towards running patient based workshops to support patients on the ASD pathway.

The post holder will identify and resolve barriers to care, working flexibly and responsively to unlock bottlenecks and delays. You will therefore be required to work autonomously and have strong organisation, communication and negotiation skills, as well as commitment and compassion for patients and their quality of care and experience.

**Key Duties and Responsibilities**:

- Develop and maintain effective working relationships with clinicians, managers and others within department and Trust wide service users.
- Be the point of contact for signposting enquires facilitating communication, responding to correspondence and resolving enquires.
- Coordinate access to relevant support channels to support parents/guardians during their child’s assessment
- Liaise with health professionals and services as appropriate.
- Liaise with parents/guardians and other consultants’ secretaries.
- Answer incoming calls using good communication skills, and appropriate tools and procedures, liaising as appropriate with the clinicians when appropriate
- Provide basic telephone advice and sign-post to other sources of support
- Take an active part in Team Meetings and audits as required.
- Coordinate patient questionnaires.
- Prioritise own workload.
- Assist in the Peer review process
- To work within safeguarding children policies and framework
- To operate within the trysts infection control policies and procedures
- To understand and work within governance frameworks
