**JOB TITLE**: Care Co-ordinator

**ACCOUNTABLE TO**: Senior Care Coordinator / Mubin Ahmed Deputy Care Manager

**SALARY**: Agreed £19,000 for the first 6 months’ probation (Upon successful appointment to start from £21,000 pa)

**ANNUAL LEAVE**: 21 days per annum plus 8 Bank Holidays

**HOURS OF WORK**: 9-5 Monday to Friday (Inc. out of hours mobile on rotational basis)

BASIC SKILLS REQUIREMENTS (including numeracy, literacy, and English-speaking abilities:
Experience in Care Coordinating minimum of 1 year

Knowledge and understanding of Dols/Mental Capacity Act/Safeguarding

Preferably NVQ level 2 or similar qualifications

Good written and oral communication skills including an ability to be sensitive to the needs of clients with mental health issues and an ability to work collaboratively with a wide range of people including Social Workers, practitioners, and other professionals.

Good IT skills including proven experience of using Microsoft Office, particularly Word, Excel and PowerPoint

**MANAGEMENT RESPONSIBILITIES**:
**General responsibilities of the position include**:
1. Deploy and supervise team/carer when necessary

2. Mentor new members of staff

3. Support existing care staff in new skills

4. Visit and provide a high standard of care in an efficient, kind, and respectful manner, preserving the dignity and self respect of the client at all times in emergency situations

5. Maintain a smart and tidy appearance and always wear the appropriate clothing whilst on duty

6. Complete a report for each client when taking lead in relation to setting up care package and all necessary follow up tasks.

7. Attend/Lead training sessions and fire drills, as required

8. Ensure confidentially and Data Protection is adhered and no matters concerning clients or business operational are not discussed with other clients or outside the office with anyone one else.

9. Observance of Health and Safety requirements

10. Report any accidents involving either staff or clients

11. Comply with written and verbally agreed conditions and employment e.g. hours of duty, days of duty, time-keeping, timesheets etc.

12. Be prepared to do extra or other duties on occasions as requested in line with the job

Day to Day Duties

4. Report issues/challenges relating to care package especially any safeguarding matters to the Manager/s.

5. Carryout daily covers, updating staff members involved and updating Care Planner on roster and sending the revised schedule if required

Daily (3 days a week - Mon, Tue & Wed, in the absence of the other Care Coordinator covers to be undertaken by yourself)

7. Undertake regular staff supervisions via telephone and also take the opportunity when staff visit the office and put other tasks on hold unless urgent and report any issues of matters where there are any concerns raised by staff member.

8. Create new client file on drop box and care Planner and delegate information/documents to admin to create physical folder

9. Organise 6 monthly staff meeting, agenda in consultation with staff members via broadcast list, type minutes of meeting and report send to Manager/s.

11. Provide support and assistance for clients throughout the period of duty only in emergency. This includes all aspects of personal hygiene, dressing, nutrition and social needs.

14. Conduct DBS for staff members

15. Visit clients (depend on what the nature of the visit is to undertake the following).
- Monitoring care workers, how they are performing.
- Checking visiting record sheets, ensuring they are legible and CW completing them properly.
- Filling visiting record sheets to say SC has visited and client is happy etc.
- Speaking to clients to see monitor how service is being delivered?
- Complete client initial assessment, risk assessment and all accompanying documents if new client visit
- Carryout spot checks / user satisfaction survey
- Carry out client review and update risk assessment

When required (You will be informed when to start undertaking visits), to complete all paperwork within 2 weeks of visit date.
