**About the Service/Department**

This new community based support service will support adults with mental health conditions who are preparing for discharge from inpatient wards (Riverside Hospital, St Charles, Park Royal Mental Health Centre and Northwick Park) to dedicated step-down properties based in Brent, Harrow, Hillingdon, Westminster and Battersea. The service will help prepare individuals for discharge and ensure minimal delays in the transfer of care, supporting them whilst they wait for care packages and secure supported accommodation or their own property.

The service will work closely with local hospitals, discharge co-ordinators, social work teams and community partners to ensure individuals are ready for discharge and are able to integrate back into the community. The service is designed to support individuals during this transition for up to 12 weeks.

**About the role

**To work with a caseload of clients with discharge related support needs.

Some other main responsibilities of the role include:

- Assertively connecting service users to other community activities and partners to promote independence and social integration
- To liaise with professionals within the community social work teams, step-down workers and partners in EACH counselling
- To work flexibly as part of a team to ensure service coverage and that contractual obligations are met

**About you**
- Ability to deliver high quality 1:1 support services to people with a range of needs and risks
- Good knowledge of welfare benefits and housing options and legislation related to statutory duty
- Knowledge of health and safety relevant to role as a Support Worker
- An ability to undertake client-focused key working, support planning and risk assessments to maximise Service Users’ independence around a range of areas
- Good understanding of the range of issues, diagnoses and treatment relevant to the client group including mental health and step-down requirements
- Willingness to actively engage in undertaking relevant personal development and training
- Ability to travel across Brent, Harrow, Hillingdon, Westminster and Battersea
- Be warm and empathetic, whilst maintaining professional boundaries.
- Previous experience of using computer based systems to accurately record work and client details
- Ability to undertake effective time management and prioritising workload
- Knowledge and understanding of safeguarding issues and ability to address them appropriately
- Ability to work with people with a range of different support needs, aspirations for their future and associated risks
