Supported living offers an alternative to residential care and empowers individuals to choose the right amount of support - how and when they need it - so they can lead an enriching and fulfilling life. Our supported living schemes are designed specifically for people with learning disabilities, autism, or complex care needs. We are passionate about delivering the best possible care and support. Supported living allows individuals to live independently and have control over how and when they receive support. We work with individuals to set their levels of care and support and ensure their rights as a tenant are protected. Our flexible support is tailored to need, and together we create personalised support plans focusing on interests, priorities, and skills. This might involve help to find employment or a plan to live more independently in the future. Whatever the need, we are here to help. The type of support a person receives is controlled by them. From help with personal care needs and managing finances, to finding education, work, or leisure activities - each person decides

Homefield House supports six people.

**Job Overview**
- To provide care in accordance with best practice and legislative requirements, reflecting policies and procedures and agreed standards under the direction of the Registered Manager, Deputy Manager and Senior Support Worker
- To support and enable individuals to maintain skills and personal interests whilst delivering person-centred care unique to each person. This also involves supporting individuals in the community such as shopping, medical appointments, religious visits, exercise and recreation.
- To maintain current skills and undertake training and development as required from time-to-time to maintain and progress knowledge

**Role Specific Duties**:

- Ensure those we support are at the heart of care delivery and their wishes and preferences enhance their wellbeing
- To support Individuals (both male and female) with all aspects of their day to day living, enabling them to enjoy the best possible quality of life
- Compassion, good communication skills and a calm and caring manner are essential for this important role
- Ensure Care Plans and other information about how to support people are followed
- Be responsible for informing the Senior Carer of any changes in the needs of individuals
- Be responsible for promoting and safeguarding the welfare of those individuals they support

**Working with Others**
- Develop effective working relationships with other employees
- Work in cooperation with members of the multi-disciplinary teams to maximise opportunities for people
- If desired by the person supported, maintain and develop relationships with family, friends and other people important in their life

**Leading by Example**
- Seek opportunities for personal and professional growth
- Be a role model for other Support Workers and be an ambassador for the service
- Be professional, polite and reasonable at all times

**Personal Responsibilities**
- Commit to achieving the relevant qualifications commensurate with the role
- Attend statutory training and any other training as directed by management
- Understand and follow all policies and procedures relevant to the role
- Be open to learning opportunities

Hours: 37.5hrs per week. This hours are set via rota and this will involve day shifts and some weekends (including bank holidays).

**Salary**: From £10.05 per hour due to increase in April 2023.

**Experience**:

- NVQ Level 2 Health & Social Care or completion of the Care Certificate (preferred)
- 1 Year Adult Social Care Support Environment
- IT Competent
- Right to Work in the UK
