Main Duties and Responsibilities Work closely with the Compassionate Community Team Lead in developing Compassionate Community approaches in response to local need and engagement with local communities. Use experience and knowledge of palliative/ EOL/ Bereavement care in support of the Compassionate Community Team and the services/ programs that are delivered ensuing safe and effective practice. Engage and build meaningful relationships with local communities, individuals, groups, and networks, especially those who experience barriers in accessing services, to understand and respond to barriers improving outcomes for all members of the community. Plan, coordinate and facilitate a community education package that is tailored to individual communities with an aim of empowering people to have the skills, capacity, knowledge, and networks to support each other before and after death.

( To deliver such training alone or in collaboration with other hospice staff as identified necessary) To work with hospice teams to identify areas where education can be usefully provided to third sector organisations, community groups or partner organisations (e.g., Bereavement, Care of the Dying, Advanced Care Planning, Last Days Matter) Maintain good knowledge of local provision and activity that can be promoted and encouraged in line with an Asset Based Community General Responsibilities Every employee is required to: Adhere to and comply with organisational policies, procedures and guidelines at all times, including financial instructions.
