This exciting new opportunity has arisen to appoint an enthusiastic and self-motivated Safeguarding Care Coordinator to make a difference to the care and support our patients receive in Bramhall and Cheadle Hulme Primary Care Network (BaCH PCN.)

BaCH PCN comprises the 5 GP Practices in the vicinity and covers a population of 57,700.

The Safeguarding Care Coordinator will provide essential support to ensure successful delivery of our safeguarding services for both adults and children across the Primary Care Network (PCN).

The aim of the role is to work alongside the Clinical Safeguarding Leads to maintain accurate records of safeguarding information across the PCN.

The Safeguarding Care Coordinator will have a key role in monitoring the quality of safeguarding within practices and will offer day-to -day support to ensure efficient information exchange between multi
- agency safeguarding partners including health providers and local authorities.

**Main duties and responsibilities**
- Processing correspondence from external agencies pertaining to safeguarding.
- Maintaining practice safeguarding registers.
- Ensure that safeguarding alerts on patient records are in place and maintained.
- Manage any safeguarding reporting requirements.
- Sharing relevant health information with other agencies when required as part of safeguarding processes.
- Co-ordinating and supporting safeguarding meetings / MDTs.
- Liaise with the Primary Care Network (PCN) Safeguarding Leads to identify recommend and implement process improvements as required.
- Supporting audit of safeguarding activities.
- Maintain an up-to-date log of contacts / services within the safeguarding arena.
- Attend the ICB safeguarding briefings.
- Disseminating important updates from newsletters and the ICB briefings in a monthly update to all practice staff.
- Keep an up-to-date resource folder for safeguarding for each practice.
- Establish and maintain effective communication pathways with practice staff and external teams appropriate to the role.

This role is home based with regular working between GP surgeries, so a car is essential.
