**Hours: Full Time - 37.5 hrs per week (includes working every second weekend)**

**Location: Based in the office at Strathcarron Hospice (Denny), occasionally working from home and meeting and reviewing customers in Falkirk, Stirling, Cumbernauld & Kilsyth.**

**Salary: £28,500 p.a. + pension + free parking + on call payment**

**Team: You will be part of an exciting small but growing team to support our Customer Care manager.**

We are not an end of life care provider like Strathcarron Hospice, however the excellence of care provided by the Hospice is mirrored in our Creative Homecare services. Our bespoke support to customers is tailored to their individual needs of care and our staff will be fully trained and be supported by our management team.

**Homecare** **- Care Co-ordinator**

You will have previous care experience working in a personal, professional or voluntary setting.
- Support the Customer Care and Business Development Managers to respond promptly and professionally to care enquiries.
- Be proficient in developing support plans and risk assessments
- Complete outcome focussed customer assessments and reviews.
- Develop our support team by carrying out observations and training
- Maintain accurate records and compile reports relating to customer’s support
- Build meaningful relationships with customers and their families to ensure their needs are met and their support is flexible enough to help them live well at home.
- Assist with scheduling and be prepared to cover gaps at short notice by providing hands on care as required.
- Support the out of hours on-call function to ensure there is sufficient cover to deliver services and support staff with emergencies.
- Coordinate holidays for the support team.
- Carry out any other duties deemed necessary for the successful operation of the business.

**Essential Criteria**
- HNC / SVQ Level 3 in social care or equivalent
- Clean driving license with your own vehicle. You will be office based with travel between locations to support customers and staff.
- Good working knowledge of IT systems with experience of Microsoft Office and CRM software.
- Experience of developing support plans, risk assessments, reviews and supervisions
- A willingness to learn and adopt new technologies where appropriate.
- A practical, methodical approach to problem solving.
- Excellent organisational and planning skills with the ability to prioritise work tasks.
- A keen eye for detail and the ability to work accurately to deadlines.
- Self-motivated and able to work flexibly.
- Excellent written and verbal communication skills.
- Warm, patient and compassionate with a clear desire to support others
- Team player with an enthusiastic and positive ‘can do’ attitude
- A competent care professional with experience of working in the community.
- If you are not already registered with the SSSC then you will be required to do this within 3 months of starting the role.
