Vacancy Description

If so, West Berkshire Council has an exciting opportunity for you to join our Adult Social Care Service as a Social Care Practitioner.

As a Social Care Practitioner, you will play a vital role in providing high-quality care and support to adults in our community.

Reporting to the Team leader you will be responsible for:

- Undertaking assessments and reviews of individuals' care and support needs
- Developing and implementing care plans, ensuring they are person-centred and promote independence
- Coordinating and liaising with relevant professionals and agencies to ensure the best outcomes for service users
- Providing advice, guidance, and support to individuals and their families
- Monitoring and reviewing the quality-of-care provision

To excel in this role, you should have:

- NVQ Level 2 in care or similar relevant qualification
- Experience in assessing and supporting the needs of vulnerable adults
- Sound knowledge of relevant legislation and best practice in adult social care
- Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, colleagues, and external stakeholders
- Strong organisational and time management abilities, with the capacity to manage a varied workload effectively
