The trust is investing in a new Frail Elderly Assessment Unit (FEAU). This is an exciting opportunity for a Discharge Co-ordinator to join our team.

The Unit will consist of 8 beds and 6 chairs. FEAU is for patients that are frail and elderly who present to the Emergency Department, patients will be assessed comprehensively by the whole of the multidisciplinary team to perform a comprehensive geriatric assessment.

The aim of FEAU is to assess, treat and discharge patients back to their own home/Placement of Care within 48hrs to reduce the length of stay in hospital for our frail and elderly patients. We will be able to provide frail and elderly patients with rapid access to investigations and treatment without the need for hospital admission.

The Unit will also link with community teams to support the virtual ward.

Experience in working in the Health Care Sector is desirable. Excellent written and verbal communication skills are absolutely essential for this post. This post is ideally suited to someone who is driven to manage complex discharges and relish the challenge of ensuring the best outcome is achieved for each patient.

Working closely with the Medical Teams, Nurse in Charge, physio and occupational therapy teams, the Discharge Co-ordinator will be responsible under the direction of the Ward Manager in planning the discharge of patients and the seamless transition from ward to ‘home’ or 'Primary Care'.

The post holder will be expected to use the integrated discharge system to identify and prioritise hospital actions to help improve flow into the community. As a duty role, the post holder will be supporting with completion of safe to transfer forms and ensuring the consistency of discharge planning.

They will be integral to the trust in minimising delays and ensuring that hospital discharges are effective and timely.

Informal visits and discussions are welcomed and encouraged.

1. Take a pivotal role in assisting the ward co-ordinator in ensuring the systems and processes of admission and discharge planning are expedited efficiently and effectively to maintain patient flow.
2. Work closely with the Integrated Discharge team to ascertain priority and area of work
3. Promote and maintain a safe and therapeutic environment for patients, respecting their views and beliefs, on admission to the ward/discharge home.
4. Establish and maintain excellent communication, informing patient and relatives/carers of time delays and organisation of admission and discharge.
5. Maximise use of the Integrated Discharge Team system to communicate discharge progress to all external partners.
