**Job summary**

Church Lane Medical Centre have an excellent and rewarding opportunity for an Advanced Clinical Practitioner to join the Practice team.

The successful clinician will join a team of highly skilled and compassionate staff, committed to providing excellent and safe patient-centred care for all who access our services. You will be required to work autonomously in the surgery, providing a range of services such as assessment, diagnosis, treatment, telephone triage and referrals, as appropriate. You will also be required to perform home visits, therefore a current, valid UK driving licence is essential.

**Main duties of the job**

Assessing, diagnosing and treating a range of acute, non-acute and chronic medical conditions. Prioritising and triaging accordingly, making any necessary referrals for investigations in the appropriate manner and liaising with the GPs and wider practice team to deliver exceptional patient care.

**Job description**

**Job responsibilities**

Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgeries or the community/care homes as an autonomous practitioner.

Use own clinical judgement to diagnose and treat patients either via face to face consultation, clinical telephone triage or other mediums.

Make appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.

Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, Patient Group Directions (PGDs), NMC standards or referring to registered GP as necessary.

Recognise the need for and initiate where appropriate, relevant diagnostic investigations, using their skills in this area to take the correct course of action.

Understand local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults

Ensure complete and accurate documentation of every patient contact, including advice given.

To develop care plans for patients when required

Be involved with continuity of care of our patients, e.g. advanced life care planning, chronic disease management.

**Qualifications**

**Essential**
- Registered first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent.
- Non-Medical Prescriber

**Experience**

**Essential**
- Experience of working as senior clinician.
- Accountability of own role and other roles in a practitioner-led service.

**Desirable**
- Experience of having worked in Primary Care.
- Chronic disease management
- Experience in Advanced Life Care Planning and RESPECT documentation
- Ability to fit and remove subcutaneous and intrauterine contraceptive devices
