Rotherham Doncaster and South Humber (RDaSH) are working in partnership with Humber NHS Trust to deliver a community based Forensic Outreach Liaison Service (FOLS) to support people with Learning Disabilities and/or Autism who are also offenders and are in secure services out of area to move back to live safely within their local communities. The FOLS in North Lincolnshire will work in conjunction with the Specialist Community Forensic Team (SCFT) and Intensive Support Service (ISS). The NL FOLS will work in partnership with their FOLS counterparts in Humber and will have access to specialist support from the Forensic Service in RDaSH

To be part of a multi-disciplinary FOLs Service and ISS providing highly focused social and health care interventions, specifically Positive Behavioural Support (PBS) to people with Learning Disabilities and/or Autism with complex needs to live safely within their local community.

To be responsible for own caseload but work closely with the ISS and SCFT Clinical leads to ensure delivery of robust evidence based Positive Behavioural Support that delivers improved patient care and experience.

To work alongside professional colleagues in the FOLS Team to support people’s transition process from out of area to local provision, within a multi-disciplinary (MDT) model that will include attending MDT meetings both within and out of area, supporting clinical assessments and feeding back information to the MDT decisions and providing a range of Positive Behavioural Support and Psychological interventions to individuals and supporters alongside professional colleagues to enable adults with Learning Disabilities and or Autism whose behaviour is described as challenging, and who may have history of offending to live safely in their local community.
