**Prison & Community Transition Coordinator**
Based: Exeter / Hybrid Working
Hours: Full time, 37 hours per week
**Salary**: £26,462 to £29,462 (Dependent on experience)
Contract: Fulltime, Part time / Flexible Working
The Prison & Community Transition Coordinator (PCTC) will work closely with teams within the prisons and community to ensure a seamless transition for our clients being released from prison. On the day of release the Prison & Community Transition coordinator will collect the individual and then support them for a minimum period of 20 weeks having already supported them for a minimum of 12 weeks prior to release within prison.
The Prison & Community Transition Coordinator will carry a caseload, delivering triage and comprehensive strength-based assessments and harm reduction interventions to support with effective prison transition, building on individuals’ strengths and sources of support.
The PCTC will work closely with Housing, Drug & Alcohol and Employment providers as well as working closely with The Prison & Probation service to ensure a coordinated trauma informed and person specific support offer is developed and delivered.

We are looking for applicants with experience working within the criminal justice system, who have an in-depth knowledge of this system including the National Probation Service,
Courts, Police and Prisons. Applicants will also have experience within substance misuse, supporting vulnerable individuals through recovery. The PCTC will work closely with other agencies and have experience of partnership work and will be committed to multi
- disciplinary working and sharing information in the best interest of personal care. A strong communicator who builds rapport quickly to develop constructive therapeutic alliances whilst maintaining professional boundaries is a must, as is an understanding of caseload management and carrying out assessments and presenting reports
