There are exciting opportunities for Qualified PWPs (Psychological Wellbeing Practitioners) to be part of the Primary Care Health and Wellbeing Team and / or join our existing dynamic core IAPT Service. Already offering psychological interventions to over 13,000 people in Sheffield each year; Sheffield IAPT is proud to have maintained our Primary Care GP base for delivering much of our work. This is complimented by an extensive range of groups and short courses that routinely run in Sheffield. The new Health and Wellbeing Team will further develop and deliver a range of adapted psychological interventions in primary care, within and across physical health condition pathways.

The post-holder will work within the Sheffield IAPT service providing high volume low intensity interventions which will be a range of cognitive Behavioural therapy (CBT) based self-management interventions to service to clients with mild to moderate anxiety and depression.

The post holder will work with people with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities

Successful applicants can access specialist top up training in long term Health Conditions if you are appointed to the Health and Wellbeing team.

This post holder will already be qualified to provide these interventions - this is not a training post.

Sheffield is at the forefront of transforming access to psychological therapies and providing integrated care for people with long term physical health conditions/symptoms alongside anxiety or depression. Our Primary Care Health and Wellbeing Team brings together experienced IAPT psychological therapists/practitioners and clinical health psychologists - to work closely with health; social care; and third sector colleagues across primary and secondary care in a range of physical health condition pathways (e.g. Diabetes, CHD, CFS/ME, IBS, Pain) to develop and deliver a range of evidence based stepped care interventions for people living with long term physical health conditions/symptoms.

We take enormous satisfaction from our track record of both retaining staff; as well as helping to develop individuals who have subsequently taken up leadership opportunities; in addition, we actively support the wellbeing of our staff.

To work as part of the wider IAPT and Primary Care Teams to provide high quality step 2 high volume low, intensity interventions to people experiencing depression and or anxiety. The interventions could be delivered using a variety of methods including face to face, by telephone, in groups or workshops.

To meet patient needs by working across Sheffield according to demand. Working a minimum of 22.5 hours per week and to offer one early appointment and one late appointment per week (outside 9-5pm working hours) and Saturday working on occasion if required.

To be worked over a minimum of 3 days which is to include Thursday afternoons as this is service wide non-clinical time for you to be able to contribute to profession specific and service wide meetings, forums and masterclasses to support you in your role and CPD.

CLINICAL

Accept referrals via agreed protocols within the service.

Assess and supports people with a common mental health problem in the self-management of their recovery.

Undertake patient-centered interviews which identify areas where the person wishes to see change and or recovery and makes an accurate assessment of risk to self and others.

Make decisions on suitability of new referrals, adhering to the department’s referral protocols, refers unsuitable clients on to the relevant service or back to the referral agent as necessary or steps-up the person’s treatment to high intensity psychological therapy.

Provide a range of information and support for evidence based high-volume low-intensity psychological treatments. This may include guided self-help computerised CBT, information about pharmacological treatments. This work may be face to face, telephone or via other media.

To complete all mandated session by session outcome measures.

Educate and involve family members and others in treatment as necessary.

Adhere to an agreed activity contract relating to the overall number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient.

Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate.
