Pennine Care NHS Foundation Trust is delighted to develop the new Community Rehabilitation Team to support service users experiencing complex psychosis. The team will support service users who are ready to step down from inpatient facilities to support them in moving to either funded placements or their own tenancy. The aim of the team is to reduce the length of inpatient stay for service users from both acute and rehabilitation wards, incorporating least restrictive practice, the NICE guidelines for complex psychosis and the Getting It Right First Time (GIRFT) national program.

We are looking to recruit a motivated and enthusiastic Social Worker to join this exciting endeavour.

Your personal development priorities will be shared and supported through our appraisal process.

We will support you in maintaining a work-life balance and whilst there is some opportunity for flexible working, the team will be based at Laurence House on the Birch Hill Hospital site in Rochdale.

The team operates during office hours of Monday to Friday 9am-5pm.

To work with a caseload of service users experiencing complex psychosis who have drug and alcohol difficulties.

To be a specialist resource for the Community Rehabilitation Team, providing expert guidance and support for the MDT regarding dual diagnosis.

To establish links with a range of services to include statutory and third sector agencies offering drug and alcohol support.

To contribute to case formulation based on the most up to date evidence based practice.

- To be part of the Community Rehabilitation Team, working with a

caseload of clients with complex psychosis and drug and alcohol problems.
- To act as a specialist resource for other within the team who will be

working with this client group
- To provide assessment, management and interventions for these clients.

**Person Specification**:

- Relevant professional qualification as RMN/OT/Social Worker
- Evidence of training (e.g.NVQ Level 3 Community Health or Drug and Alcohol Use Practitioner course) or relevant experience

of working in the mental health and drug and alcohol field
