We are looking to recruit an experienced Care Coordinator. You will be based at our Penwortham office and will be working closely with our Registered Manager. Willows Home Care provides quality care to people in their own homes.

You will liaise with external agencies such as Hospitals, GPs and Social Services, as well as clients family members. You will meet new clients/families and coordinate a care package, risk assessments and care plan.

You will manage a team of Care Workers, taking responsibility for dealing with performance issues, and carrying out staff supervision and appraisals. The role involves on-call responsibility and weekend work on a rota basis. A full driver's licence and access to a vehicle is essential for this role along with good IT skills and ability to work within a fast-paced environment to deadlines. You must be an organised individual and have excellent time management skills.

Duties and Responsibilities:
- To cover all calls on our electronic rota system and ensure weekly rotas are completed in a timely manner.

To follow up the initial contact between the care worker and clients to evaluate the Care Plan and ongoing needs.
- You will be responsible for completing quality audit checks by telephone and face to face meetings with our clients/families.

To maintain accurate written and computerised record keeping you will be required to participate in an on-call rota ensuring 24-hour cover.

**REQUIREMENTS**:

- domiciliary care experience,
- a minimum of one year experience as a care-coordinator
- experience using a computer based rostering system
- driving licence and access to a reliable vehicle
- flexibility
- excellent time management and organisational skills

**Salary**: £25,000.00-£28,000.00 per year
