An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the South Islington Rehab and Recovery Team based. As a valued member of a well established and supportive multi-disciplinary mental health team you will be delivering holistic care and support alongside adult service users as part of their recovery from episodes of psychosis and in managing long term conditions.

As part of the clinical team you will be joining doctors, nurses, psychologists, social workers, occupational therapists and employment and welfare rights specialists in a collaborative and supportive environment that provides care and support with a focus on engagement and social inclusion.

The ability to work on your own initiative is vital as well as embedding yourself within the team approach to maximise the effectiveness for yourself and the team.

Due to the nature of the client group we see, on-going risk assessment is an intrinsic part of the working day. A robust supervision structure is in place and any necessary training will be provided through the Trust’s Learning & Development team.

Reporting to the team manager or the nominated deputy, you will be
responsible for participating and assisting in the delivery of assessment, health and social care services to service users as part of the multi-disciplinary team approach. You will be expected carry out a range of practical tasks, including direct patient care, as identified as part of the service users’ care plans and pathways.

You will provide direct support to an allocated group of service users as
delegated by the Team manager or a senior member of staff. You may, under supervision, take on a key worker role or specific elements of care for a designated group of service users or carers, and also be part of our intake service providing signposting and advice.
