**Main purpose of the post**:
The Complex Disability Day Service aims to provide high quality care for those people who require safe stimulating environments and supervision which is carried out by appropriate trained staff that are able to meet their physical and emotional needs.

The service will operate Monday - Friday to provide a schedule of activities and therapies in conjunction where appropriate with health professional support which will be determined by their individual Care Plans.

The opportunities and activities facilitated by the service aim to maintain and improve people’s health and wellbeing as well as providing much needed respite for families.

The service will support people to live, contribute and develop valued relationships within their communities that; encourage naturally occurring support networks.

**Main duties and responsibilities**:
1) To actively support service users, engaging service users in activities ensuring that such activities are valued and appropriate to meet their individual outcomes.

2) You will be expected to effectively manage a complex range of individual behaviours in line with detailed Positive Behaviour Management Plans.

3) To support people in accessing activities, therapies and outcomes as identified in their Care Plan and appropriate to their needs.

4) To provide manual handling support and mobility assistance to people, with regards to personal care and mobility needs.

5) To administer and assist service users with medication in accordance with the Neath Port Talbot Medication Policy.

6) Preparation, serving of food and supported eating and drinking with people in line with their own personal plan of support.

7) Laundry including use of domestic laundry equipment.

8) Effectively assess individual needs and deliver an appropriate service, under the guidance of the Keyworker, Service Coordinator and Manager.

9) Contribute to the development of the service and be actively involved and have a full commitment to the ongoing development of the service.

10) To have an underpinning knowledge and commitment of the service users within the service setting, who may have a wide range of communication barriers, and in partnership with our Health Colleagues, look to develop ways of overcoming these barriers.

11) To have an underpinning knowledge and commitment to the Continuing Health Care needs of each individual who attends the service, working in partnership with our Health Colleagues.

12) To work in partnership with care providers, sharing their knowledge of the individual to help improve service delivery.

13) To work in accordance with all policies of Neath Port Talbot County Borough Council, service documentation and the standards of professional conduct and practice required of those employed in the social care profession in Wales.

14) To liaise with carers, colleagues and other agencies when necessary.

15) To report back to the Manager, Care coordinator, and keyworker any concerns, worries or any relevant information regarding the service users’ physical and mental health.

16) To support the Management Team in developing a flexible, caring and sensitive approach to service delivery.

17) To report any concerns relating to individuals you may support, in line with the authorities Protection of Vulnerable Adults procedure.

18) To attend and contribute at regular team meetings.

19) To undertake any training deemed necessary for the improvement and development of staff and for the service.

20) To undertake and participate in regular supervision and annual appraisal with the Manager / Care co-ordinator.

21) To be responsive to changes in service user needs and to adopt a flexible working pattern to accommodate such changes to the service demands as deemed necessary by the Manager/ Care co-ordinator

22) To have access to suitable transport enabling you to support service users on activities/sessions or any other work related business.

23) To undertake any duties as directed by Line management appropriate to grade and role.

24) To be responsible for adhering to the risk assessments and manual handling plans. To take responsibility for their own health and safety and to that of others within the HSWA and report any potential hazards and risks to the Manager / Care co-ordinator.
