**Contract/Hours: Permanent / Full Time - 37 hours per week** (4 week rolling rota including; days, evenings and some weekends.)**

**A bit about the role**

**Our East Lothian New Horizons Service** offers residential housing support to young people aged 16-25 years who are referred to us and are either declared homeless or threatened with homelessness. Our service aims to empower young individuals, helping them prepare for more independent living. We work closely with residents on various essential life skills, including budgeting, food preparation, safe storage, household chores, practical skills, managing neighborhood relations, and resolving disputes. Additionally, we facilitate access to specialist services and focus on employability.

We are currently seeking a **Housing Support Practitioner** who can engage with young people. As part of our team, you'll provide structured and focused housing support, enabling these individuals to gain crucial life skills. Whether through one-on-one sessions or small group work, our goal is to help them live more independently and sustain their accommodation within the community.

You'll join a committed and dedicated team who together help build a foundation for a brighter and more secure future for the young people we work with.

**How you'll help to create brighter futures**

Some key responsibilities for the Housing Support Practitioner role are;
- Building working relationships with multi-agency partners from the statutory and voluntary sectors to deliver the best outcomes for young people, including ELC Housing Department, benefits agencies, specialist health services, employment and training providers.
- Supporting young people to engage with local agencies and organisations which assists them to sustain their accommodation and place within the community e.g. accessing leisure activities, GP and health services.
- Ensuring that the Service complies with all Action for Children and East Lothian Council Standards and Procedures and with regard to case file recording / paperwork, incident and accident reporting, safeguarding and health and safety. Practitioners will use Action for Children E-systems to identify needs, interventions and the Outcome Star to measure and record outcomes.

**Let's talk about you**
- SVQ3 or equivalent or a desire to work towards this in the first year of employment, to enable registration to the SSSC as a worker in Housing Support.
- Ability to follow procedures and complete assessments, develop plans and review plans to ensure positive outcomes.
- A knowledge of an outcome based approach to planning, delivering and reviewing services.
- Producing concise and accurate reports to agreed standards, formats and timelines, which may be used ultimately for formal reviews.
