**A bit about the role**
Do you have a sound understanding of issues affecting young people and families, maybe through experience working with care experienced young people and families or through your own lived experience?
Chances Service works with vulnerable young people aged 12- 21 for whom there is significant professional concerns and elevated risk factors. Chances Service offers direct work with vulnerable young people focusing upon their behaviour, building on their strengths and qualities, strengthening family links and engaging positively with their community.
You'll deliver tailored support through varied interventions consisting of weekly 1-1 sessions and pro-social activities in various community settings to young people who have been referred to the service. Intervention support is on a flexible hours basis and includes evening and weekend support depending on the young person's plan.
You'll act as a role model, be a consistent positive adult and focus on building relationships with young people and their families that will ultimately help divert behaviours, build structure and confidence to achieve positive outcomes based on an assessment of their needs. It is imperative the post holder works alongside the family to provide support, address vulnerabilities and strengthen family relationships.

You'll play a key role as part of a wider team of professionals across multi-agencies and it will enable you to grow your experience and build your network with a wide range of organisations across the region and gain invaluable experience within a great charity.
Key responsibilities will be;
- Providing direct community based 1-1 to children and young people referred to the service and wider working with the family.
- Providing community based interventions to divert children and young people from the concerning trajectory their behaviour is placing them on.
- Maintaining your own calendar, keeping up to do case notes and sharing significant information with the child's key team where necessary.
- Working together with East Lothian Children's Services on agreed children's plans and co-producing solutions.
- Working directly with the family and ensure that the family has a voice throughout the period of involvement with the service;
- Working in partnership with other teams/organisations who may be involved in the young person's care plan to deliver outcomes;
**Let's talk about you**
You'd ideally have the below in order to help succeed in this role, however we are very open to relevant experience.
- An SVQ 3 in Health and Social Care or equivalent is desirable.
- Proven assessment skills and experience of working towards outcome focused care / service plans.
- Have the ability to communicate effectively and engagingly in order to maintain independent, professional relationships.
- Utilize management systems and databases.
- Driving licence and access to a car. The job requires the ability to travel according to the needs of the job, with reasonable adjustments, if required, according to the Equality Act.
