Our fabulous #TeamTransitions are looking for dedicated and understanding individuals to join our Family and Transitions team in North Lanarkshire as Support Practitioners. We have contracts available up to 36 hours per week.
**The Role**
As a** Support Practitioner** you’ll support children and young people in all aspects of their lives as they make the transition from childhood to living independently and make plans for their future. You will support them to become part of their local community and take part in various activities from horse riding and swimming to working towards their Duke of Edinburgh award, weekends away for respite. You’ll support them to build upon their independent living skills and all aspects of daily living, including cooking and managing their finances. Supporting with their personal care and medication needs are also key parts of this role.
The children and young people we support come from diverse family backgrounds. Some of the individuals may have been affected by their own, or another’s, alcohol or substance misuse, or poor mental health and wellbeing. Our team provides essential support within these families to strengthen their own coping skills and strategies, and are occasionally required to provide to crisis supports.
As travel will be required throughout North Lanarkshire, it is **essential** that you hold **a full, clean UK driving licence **and have access to your own vehicle.
To be a great **Support Practitioner**, you'll be:
- empathetic with a warm, friendly personality and a positive, can-do attitude
- able to demonstrate exceptional communication skills and patience
- able to think on your feet and use your own initiative
- fully aware of and understand the duties and responsibilities in relation to child protection and safeguarding
- creative in meeting the needs of the children and young people we support
- competent in the use of technology to enhance the support you provide
- confident in managing behaviours that challenge using Positive Behaviour Support strategies
**Shift Patterns**
We provide support 7 days a week which is tailored to the needs of each individual. There will be a mix of shorter shifts and longer blocks of support and shift patterns will fall into patterns of early mornings, days, evenings, sleepovers, weekends and public holidays as part of your rota.
**Our sleepovers are not included in your contracted hours, so you’ll receive an additional sleepover payment of £105.00 per night for each sleepover shift worked.**

If you’re successful in joining our team, you will be expected to join the Protecting Vulnerable Groups (PVG) Scheme and must be willing to work towards a SVQ in Social Services and Healthcare as required for SSSC registration. We will need you to register with the SSSC within 6 months of starting your employment with us.
