Care Co-ordinator

Working within the policies set out by the company to coordinate the provision of care to ensure high-quality practical support and encouragement to those who would not otherwise manage satisfactorily in their own homes.
**Main Duties**
1. Effectively deploy the home care staff in accordance with the daily schedule by communicating closely with them, the clients, and the clients’ families.
2. Ensure that care staff fully understand the assignment, their tasks, and the start time and duration of each visit.
3. For new assignments, ensure that care staff understands the client’s needs, that the arrangements are confirmed with the client and a Service Delivery Pack is provided.
4. Help monitor the suitability and effectiveness of the service provided by communicating regularly with the clients and care staff. Completing quality checks and updating the EMS system.
5. Ensure that an effective, high-quality service is provided by communicating and working closely with all colleagues and others involved in the client’s care.
6. Receive and help administer inquiries, new instructions, changes to the program, queries from care staff, requests for information etc.
7. Maintain records and help develop administrative systems as required by the company and to ensure compliance in a highly regulated environment.
8. Participate in training and in the training, support and supervision of others.
9. Personally deliver care not only as a back up but as a means of supervision, keeping in close contact with the clients and in supporting the care team.
10. Show initiative in obtaining a thorough knowledge of the Company’s Equal Opportunities and Health and Safety policies and working with others to ensure their implementation.
11. Attend to client reviews and assist in the investigation of any complaints.
12. Carry out other duties as required and commensurate with a small team in a rapidly developing environment.

- A systematic yet flexible approach ensuring accuracy and attention to detail.
- A knowledge of the Bristol, Bath and South Gloucestershire areas is desirable.
- Car driver an advantage.
