You will be responsible for the oversight and management of the Transfer of Care Hub (ToCH) within the Acute In-reach & Onward Flow Cluster of the Community Division.

Across Hampshire and the Isle of Wight healthcare services are evolving and transforming to meet the continuing care needs of our population. Now is a very exciting time to join Isle of Wight NHS Trust as we enter this period of change.

This year Isle of Wight NHS Trust and Portsmouth Hospitals University NHS Trust have strengthened our partnership and formed a group giving us an opportunity to work together to be innovative in improving patient care and outcomes as well as improving the experience and opportunities for our people across the two organisations.

We have also been working with other local NHS Trusts across our region to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the new organisation will be called Hampshire and Isle of Wight Healthcare NHS Foundation Trust.

As part of the move towards working as one organisation, people employed in our community, mental health and learning disability services by the Isle of Wight NHS Trust on 30 April 2024, will transfer to Southern Health Foundation Trust under Transfer of Undertakings (Protection of Employment) on 1 May 2024.

On 1 st July 2024 Southern Health will become the new organisation (Hampshire and Isle of Wight Healthcare Foundation Trust).

The Isle of Wight NHS Trust will continue to deliver both acute and ambulance services for the island population.

**Key Responsibilities**:
The post holder is expected to work autonomously using own initiative to provide comprehensive, efficient and effective management of the service ensuring regular communication with team members.

To provide focus and support to the Service Lead on objectives - performance, governance, workforce, service improvement, information management, communications, finance and risk.

To be responsible for the strategic development, implementation and management of ToCH policies and procedures, working autonomously and using own initiative.

To lead on collection, collation, analysis and interpretation of complex performance data and audit reports within the service as requested ensuring they reflect changes in data collection requirements.

To ensure that professional and own behaviour is managed in line with the Behavioural Framework competencies.

To seek improvement, where opportunities exist providing co-ordination of activities to support change

To act as line manager for staff within the ToCH Business team and lead management of the service, using own initiative to resolve problems as they arise including, supervision, recruitment, appraisals, performance issues and initial stages of grievance and disciplinary procedures.

Supporting development of service integration across the Discharge Pathways.

Use own initiative to work in an autonomous manner to provide a high-level comprehensive, confidential service. The post holder must be visible, accessible and provide an authoritative presence/role model within the team to whom staff can turn to for assistance, advice and support.

To support the effective rostering of staff on HR systems in accordance with divisional or trust-wide policy.

To establish systems and processes to report and monitor performance for key targets (local and national) and operational objectives. To be solution focused resolving problems where performance is not achieved with the support of the Service Lead

To lead on collection, collation, analysis and interpretation of complex performance data and audits producing complex reports and spreadsheets using advanced IT skills, resolving errors and interpreting data.

To support the Cluster Lead, Service Lead and BSO in the effective co-ordination of the divisional governance framework, including the co-ordination of performance and audit reports, evidence of quality compliance (including information management, mandatory training levels, business continuity and general policy compliance) and effective risk management.

To ensure the smooth flow of all incoming and outgoing correspondence

To support the Service Lead in the maintenance of staff and patient environments to ensure all areas of ToCH are fully utilised, asset registers maintained with a pro-active approach to improving work environments for all.

Any other support including business/administrative duties required by the BSO, Service Lead and Cluster Lead
