**About the role.**
In this role, you will play a key part in the delivering the Enablement Activity Based Care Programme, which provides person-centred care following individual enablement plans, relating to individual vulnerable people with a wide range of complex care and behaviour support needs. You will be delivering a variety of community and building based activities, geared towards the individual’s needs and aspirations -offering a high level of support to each service user, in accordance with their personal care and support plan. You will be able to support the individual to achieve agreed enablement goals, gain new or improved skills promote confidence and community engagement alongside encouraging new experiences.
You will assist in the planning, implementation, monitoring and regular reviewing of the care and enablement goals and behaviour support plans, by being actively involved with service user, carers, ABC Manger, ABC officers and a range of other key people involved the service users care and support. Also actively participating in team meetings, supervisions, Achievements and Development meeting assisting to further develop the service.
You will be responsible for the safe handling of medication, ensuring all medication is administered as prescribed and in line with policy, seeking advice and guidance as needed, helping to safeguard the individual in your care and supporting your colleagues to fulfil their roles.
You will be expected to complete administrative duties within the service ensuring records are maintained to a high standard. You will be expected to communicate effectively with colleagues, cares and other professionals in order to share knowledge, seek advice and co-ordinate effective service delivery.
You will deliver good quality, safe working practices and adhere to the quality assurance processes of the service, adhering to the established Quality Assurance Framework and best practice. Accurately recording and reporting any concerns or changes to the Service user’s health and well-being, including any behaviours of concern. Providing written reports as necessary top contribute towards the effective care planning and enablement goal setting arrangements and reviews.
You will be expected to work in a flexible way to ensure that all work commitments are fulfilled, meeting the changing needs of the service, to work flexible hours when required which may include early mornings, evening and weekends.
