Band 7 - MDT Coordinator (clinical)

The post holder will undertake a lead clinical role in the MDT and Integration Transformation Programme across Specialist Children and Young People’s Services (SCYPS) and our system partner services in Newham.

Working with Paediatricians, Nurses and Therapists, the post holder will be responsible for co-designing and implementing integrated care pathways delivered within SCYPS and with system partners, such as acute, universal services (provided by London Borough of Newham and Primary Care), education and social care.

The Integrated Care Coordinator Lead will lead and support the delivery of Integrated Care and influence service development.
- To ensure smooth running and through put of all communication processes and correspondence within SCYPS MDT and systems integrated working.
- With the support of clinical leads, develop processes to effectively review, coordinate and distribute referrals, taking a MDT perspective on timing and prioritising of the most clinically appropriate assessments and interventions for children with complex disorders.
- To manage Integrated Care (MDT) meetings, associated preparation and facilitation. The post
- holder will work flexibly to meet the demands of the service.
- Coordination of Health Professionals outside of the core/ specialist services for input into patient care or attendance of MDT.
- Management of incidents and complaints related to MDT working.
- To ensure the interface between the acute hospitals, primary care, community health services, social care, housing, the independent sector, voluntary sector are robust at all times.
- To maximise effective patient care through the development of MDT and integrated services care coordination pathways
- To ensure that MDT and integrated care pathways are responsive to the needs of local population.
- To engage with other health and social care systems around good practice for integrated care and research models of service.
- Ensure our integrated pathways provide high quality care appropriate to client needs.
