We are currently seeking to recruit a Team Leader to join our Gateway to Care Team. Receiving over 400 contacts a day, Gateway to Care is a busy team supporting people at the initial point of contact ensuring appropriate signposting, direct access to services or referral to appropriate teams in a timely and proportionate manner.

We have a clear vision of our role in supporting people in Calderdale to maximise choice, control, and independence. The team currently comprises of qualified Social Workers, Nurses, Occupational Therapists and Social Care Advisors. We pride ourselves in an integrated, solution focused and strength-based approach which engages partners to secure better outcomes and optimise networks of support.

The role of Team leader is to take the lead in the day to day operational management of the Team, including allocation of referrals, supervision and workload management, collaboratively with the Team Manager to drive change, to monitor attendance and performance and agree any delegated duties.

Ensure that safeguarding adults at risk is the core of service delivery.

Partnership working is an essential and critical part of the role. The post holder will be required to work collaboratively with a wide range of partners including Statutory Organisations and the 3rd Sector to deliver outcomes for people in Calderdale.

This role will have no caseload of their own.

You will be responsible for:
Social Workers - Newly Qualified to Level 3

Service Co-ordinators - Scale 5

Other Health and Social Care staff as needed

Please note, this role is Monday to Friday between the hours of 09:00-17:30. You will be required to work a minimum of 3 days in the office.

We are based at The Shay Stadium, Shaw Hill, Halifax HX1 2YS and have parking outside
