We are currently looking to recruit an enthusiastic and motivated Band 4 assistant practitioner to join the therapy team 22.5 hrs per week. You will be an integral member of our multi-disciplinary community based team working in Pocklington. You will be expected to deliver clinical care delegated by a registered professional and contributing to the assessment and evaluation of person centre care plans.

The role requires excellent communication skills, IT skills, and the ability to be adaptable, reliable and highly organised. Already established relevant community clinical competencies are desirable for this post.

The ability to commute within the Trusts geographical area with access to the appropriate means of transport is essential.

Working within the therapy team to assist the rehabilitation of our patients. This will include assessments, equipment provisions, exercise plans, support with any urgent response patients to prevent hospital admissions.
