Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the Complex Discharge Team, Ward Multi-Disciplinary Team (MDT) and Community partners, constructively challenging where appropriate, decisions with regards to discharge planning.
To undertake administrative tasks to support baseline assessments in partnership with other members of the MDT and ensure that the patient is kept informed of their discharge plans at all times.
Represent the Integrated discharge team at appropriate meetings.
Provide effective & timely communication of discharge actions using existing communication tools.
Will be expected to undertake a range of duties without direct supervision but will be required to report back on those delegated duties to the ward team and case manager.
Will be competent to work across multi
- professional disciplines and make referrals for additional assessments and treatments as required, supported by the case manager.

An exciting opportunity has arisen for an enthusiastic innovative, motivated and dynamic Administrator who will be working as part of a multi-professional Team to support the smooth running and co-ordination of discharge services.
The post holder will be responsible for providing administrative tasks to support facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate assessments of patients needed and support the discharge plan process. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Integrated Discharge Team to overcome and limit these.
This role will extend to support and ensure that patients who have been discharged feel supported and have received the support that was requested. You will need to have critical thinking skills and be able to communicate effectively. This role will be full time across 7 days so flexibility will be required.

Croydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon.

- Croydon University Hospital
- Purley Hospital
- The Sickle Cell and Thalassaemia Centre in Thornton Heath
- Community bases at Purley, Sanderstead, New Addington, Broad Green, Woodside
- A minor injuries unit in New Addington
