Our client is looking to recruit a passionate and solution focussed Case Management Team Manager at HMP Pentonville.

**Job Role**

As Case Management Team Manager, in the Early Days in Custody (EDiC) Team, you will oversee the effectiveness of the case management system, within the substance misuse service. The EDiC multi-disciplinary team supports individuals during the first 14 dayswithin prison.
- You will draw on expertise to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed
- You will support and lead Recovery Workers, ensuring they provide initial support, continuity and coordination for people who use their service
- You will ensure that individuals are accessing the relevant services available in the prison and that there is a plan in place for each prisoner to support their long-term engagement with healthcare and the management of any health condition they mighthave

This role will be rota-based working early and late shifts, and Monday-Saturdays on a rolling rota basis.

**About You**

As an individual, the company are looking for someone who is self-motivated and good problem solver who can work on their own initiative and as part of a team.

You will need excellent communication skills and the ability to navigate challenging situations with a solution focused approach.
