An exciting opportunity has arisen for a ward clerk on Ward B3 Halton, at Warrington & Halton Teaching Hospitals NHS Foundation Trust, (Halton Site).

Ward B3 is a busy, fast pace Ready for Discharge Ward with an inpatient bed base comprising of 27 beds, maintaining the safety of patients who are medically optimised for discharge but have complex discharge needs.

Applicants should have excellent communication skills, the ability to prioritise own workload, and work well as part of a team

Ward B3 are looking for someone with previous clerical experience (Hospital experience preferred but not essential).

B3 ward clerk must have an excellent telephone manor speaking to relatives and other health care professionals via the telephone and passing on confidential information to the relevant members of staff.

Must be able to organise own work load ensuring all duties complete within a reasonable time frame.

Main Tasks and Responsibilities
1. Receive patients on admission, promoting a pleasantly receptive atmosphere. Ensure case notes are available, filling administration documents as necessary.
2. Maintain case notes and in good order. Ensure all case notes are appropriately tracked electronically.
3. work with other members of the multi-disciplinary team as required.
4. Maintain and update the bed statement. Keep an accurate record of admissions, transfers and discharges utilising the computerised administration system.
6. File Documents and reports in case notes as per trust policy.
7. Obtain outpatient Appointments liaising with appropriate department. At the same time advise of transport requirements.
8. Arrange transport when required for patients.
9. Assist with the dispatch of discharge letters etc. to primary care providers to ensure seamless discharge of patients from secondary to primary care.
10. Complete information bulletins recording admissions, transfers, discharges and change of addresses, requests for labels etc.
11. Answer the telephone and general enquires in a courteous manner, to the ward/ department, seeking advice where appropriate.
12. Convey messages and urgent documentation between the ward and departments as necessary.
13. Report immediately to the Senior Nurse / Manager on duty any accident, complaint or untoward incident involving staff, patients, or visitors in accordance with Trust Policy.
14. Attend as directed in-service training / instruction adopting a positive attitude to your development.
15. Be responsible for ensuring that information scanned into Lorenzo records are timely and accurate.
