Job description

PSALT is an NHS organisation that offers OST prescribing for people dependent on opiates such as heroin. We work to stabilise people on methadone or buprenorphine and reduce illicit drug use. **Duties and Responsibilities**

Manage a case load of patients involving monitoring, assessing and motivating individuals during their treatment.

Design and implement care management plans with patients. Ensuring the monitoring and updating of the plans so that outcomes are recorded correctly.

Carrying out three monthly assessments and in consultation with each individual, make appropriate recommendations to their clinician regarding their treatment, and follow up on any agreed interventions and record appropriate outcomes.

Sign post or refer to other services where by the need of the patient cannot currently be met by the PSALT programme and keep accurate records in relation to this.

Deliver harm reduction information and advice.

Monitor and manage the attendance of patients.

Maintain accurate patient records and information using the WCCIS system..

To ensure that any recommendations following an assessment, review or drop in are recorded in the appropriate format and to ensure that those recommendations if agreed are adopted.

To ensure that any significant events are recorded and logged in the appropriate manner.

To provide a monthly statistical report to the project manager

To ensure that a professional and competent health service to all PSALT patients in line with PSALT protocols and procedures.

To attend any meeting, training or conferences as required.

To undergo supervision on a regular basis.

**Experience**

Previous experience in a similar supportive role is essential.

Previous experience of working in a confidential service is an advantage but not essential

Previous experience of delivering behavioural change interventions in an advantage but not essential

**Job Type**: Full time (35 hours Monday to Friday)
