Hospital at Home is expanding its workforce due to ongoing success since its inception during covid. The service has evolved to meet increased need and changing patient and clinical priorities. To support this we are developing our workforce model and clinical lead therapists will be a part of this. This is an exciting opportunity for experienced community therapists to progress further in their clinical career pathway. The hospital at home service facilitates the care of patients at home where their condition can be managed appropriately and safely within the home and the patient supports this approach.

This post holder will support the service lead to drive and sustain performance, delivery of high value, high quality health care.

The post carries the responsibility of continuity of clinical leadership and management within the wider hospital at home team in the care coordination hub and the team working in therapy triage which is a 7 day a week service based at Robertson House, Stevenage.

The post holder will act as a role model for staff, development, teaching and training is essential.

The post holder will ensure a high standard of family-based care is achieved and maintained by the whole team and has responsibility for overseeing the quality and effectiveness of the assessment and planning, of individualized care to patients being.

To hold responsibility for own triage caseload within the hospital at home service and for the delegation and monitoring of caseloads to physiotherapists, occupational therapists and triage assistants within the team, ensuring patients can receive assessments appropriate to their need and a high standard of care is achieved meeting patient expectations.

The post holder supports the management of the teams in the therapy triage team with recruitment, appraisals, performance and sickness investigations, audits and efficiency of the team.

This role will provide both a leadership and clinical function there is an expectation that post holder will retain a high level of clinical skill used both daily in the triage and management of patients on hospital at home caseload and to support the team and/or enable skill role modelling and development of staff.

The post holder will ensure a high standard of family-based care is achieved and maintained by the therapy triage team. The post holder will work autonomously demonstrating effective management and leadership skills.
