The role will include responding to variations in workload as outlined below:

- To support the virtual ward to progress the patient’s pathway thereby reducing any delays for the patient and working toward the patients Expected Discharge Date.
- To follow up and progress actions to deliver the timely onboarding of patients to ensure they begin their journey on the virtual ward in a timely and seamless manner.
- To meet the direct and indirect care needs of patients under the delegation and supervision of a Registered Nurse or Therapist.
- To ensure the delivery of high standards of patient care at all times, in line with Trust policies and guidelines
- To work as an effective member of the multi-disciplinary team.
- To work within the boundaries of the role and refer all other matters outside of their scope of practice to the Registered Nurse or Therapist.
- To observe high levels of confidentiality at all times
- To manage and prioritise own workload within deadlines
- To record all types of information accurately and have the ability to produce and maintain documents and spread sheets.
- To work as part of the multi-disciplinary team, ensuring effective communication across all departments.
- To communicate effectively with patients, carers, visitors, other staff members and other agencies always to ensure all are aware of pathway progression and admission/discharge plans
- To progress patient onboarding and discharge as early in the day as possible.
- To escalate to senior staff any barriers that may result in a delayed transfer of care
- Review actions and constraints at morning/afternoon huddles and identify how any blocks to progress can be addressed.
- To liaise with the nursing, medical and pharmacy teams, to ensure that all medications are prescribed and dispensed from the referring hospital before acceptance onto the Virtual ward.
- To liaise with the social services team, as directed to assist with patients needing support with their remote monitoring technology.
- To organise Doccla kits for discharge with family/carers, voluntary services.
- To undertake and/or assist with audits as required.
- To accurately document all communication with family, carers and external agencies in relation to the admission/discharge process.
- To provide administrative support to ensure the smooth running of the virtual ward, covering A/L, etc. as required.
- To monitor the clinical task box, escalating messages as required
- To progress patient onboarding and discharge as early in the day as possible.
