1.1 Communicate in a clear manner that is consistent with relevant legislation, policies, procedures and service needs. 1.2 Maintain and complete accurate records either written and / or electronic which are consistent with legislation, policies and procedures and service needs. 1.3 Communicate with a range of people on a range of matters in a form that is appropriate to them and the situation. 1.4 To escalate changes in the status of people receiving care to the registered staff member on duty 1.5 To report effectively to the Multi Disciplinary Team (MDT) as required: ad hoc, in handover and during formal meetings as required.
1.6 Constructively manage barriers to communication. 1.7 Acts in a manner that acknowledges patients right to make decisions and recognises their responsibilities. 1.8 Maintains confidentiality during communication consistent with legislation and employers policies. 1.9 Communicate effectively using computer systems within the organisation.
1.10 To act as the patients advocate as required ensuring individual assessed needs, preferences and choices are delivered by the service. Addendum Liaise with and ensure good communication with statutory/voluntary bodies to establish a comprehensive package to ensure continuation of care. To provide an opportunity and respond to patients and carers to enable them to express their feelings whilst encouraging acceptance and adjustment to their new circumstances. To communicate effectively with patients and carers recognising the need for a wide range of verbal and non verbal communication methods with patients who may have difficulty in understanding or communicating and who may be low in mood and lack insight.
2.10 To recognise and understand roles and responsibilities of individual working in the Livewell Southwest. 3 Health Safety and Security 3.1 To adhere to LSW policies and procedures and promote Health and Safety at work. 3.2 To support others in maintaining health safety and security. 3.3 To identify and assess the potential risks involved in work activities procedures for self and others, discussing with the registered member of staff.
3.4 Work with relevant colleagues to identify solutions to the management of any risks identified. 3.5 To take the appropriate action to manage an emergency summoning assistance as necessary and completing accurate reporting systems. 3.6 To be responsible for the safe use of all equipment within their scope of practice. 3.7 To identify needs and advise on resources as required.
3.8 To assist in maintaining a clean environment that meets Infection Prevention & Control requirements and is conducive to safe practice. 3.9 To participate in standard LSW Health & Safety risk assessments which support the safety of staff and people who use services 3.10 Report all accidents and incidents involving self, patients, staff or visitors according to Livewell Southwest policy. 3.11 Participate in the response to untoward incidents. 4 Service Improvement 4.1 To co-ordinate the day to day activities of Band 2/3 staff where applicable.
4.2 To be responsible for collecting and reporting statistical data in line with LSW policies and procedures. 4.3 To demonstrate leadership skills and role modelling through the management of designated projects. 4.4 To contribute to the planning, evaluation and audit of practice, clinical pathways and protocols within the clinical setting. 4.5 To participate in research and clinical audit as appropriate keeping up to date with changing practices.
4.6 To alert the supervising registered staff member when direction, policies or strategies are adversely affecting users of services or the public. 4.7 To be actively involved in team effectiveness by attending staff meetings and contributing as a team player to team development. 4.8 To contribute to the non clinical day to day running of the service as necessary to ensure efficient service delivery. 4.9 To undertake specifically identified administrative and clerical tasks associated with delivering care which facilitate the efficient running of the ward/department.
4.10 To actively contribute to service development / improvement plans. 5 Quality 5.1 To contribute to the clinical governance arrangements and quality agenda. 5.2 Works within the limits of own competence and responsibility and refers issues beyond these limits to the relevant registered staff members. 5.3 To comply with legislation, policies, procedures and other quality approaches relevant to the work being undertaken.
5.4 To maintain an up to date record of all contact with people using services, MDT care plans, assessments and reports. 5.5 Ensuring confidentiality at all times in line with organisational policies and procedures. 5.6 To act responsibly as a team member and seek help/support as necessary. 5.7 To use and maintain resources efficiently and effectively and to provide evidence of these efforts.
