**Detailed job description and main responsibilities**:
1. To manage a caseload of neurodiverse clients (diagnosed or self diagnosed) and support them to gain, retain and sustain paid employment 2. To carry out a vocational assessments, to identify vocational needs and develop action plans in agreement with the individual.
3. To maintain detailed and accurate case notes, action plans and Thrive participant paperwork and upload these on to the management information system within the allocated timeframe.
9. To ensure that the service eligibility is adhered to, providing information on alternative employment support services for those who are not eligible, and monitoring progress and outcomes of those supported by Thrive ensuring that stringent monitoring deadlines are adhered to, and KPI evidence is gained regularly. 10.To liaise and communicate regularly with the referrer and other relevant healthcare staff to ensure that all parties are kept informed and coooperate in helping the individual achieve and sustain

Essential criteria:
Educated to degree level or equivalent
Able to build relationships with participants and partners
Good report writing skills
Experience of supporting people with health conditions and other barriers into work
Desirable criteria
L4 IAG
Vocational assessment skills
