We are looking to recruit a motivated and highly skilled Enhanced Clinical Practitioner to join the medical team on our inpatient rehabilitation ward at Wimborne hospital. You will be working within a dynamic and supportive multidisciplinary team to provide high quality person centred care to support our patients rehabilitation pathway within the community hospital setting.

You will be based on Hanham ward which specialises in older people's rehabilitation but also cares for patients with other long term and sometimes complex medical needs. The post holder will provide clinical advice, leadership and support to the ward staff, also ensuring the needs of the service are met by exercising and demonstrating high levels of autonomous clinical assessment, critical analysis, and decision-making skills to deliver high quality patient centred care. This clinical expertise will form an integral part of the daily medical management and discharge planning for our patients.

The current shift pattern for this service provision is weekdays.

To provide high quality person centred care which always considers people’s safety, privacy and dignity underpinned by enhanced clinical knowledge that is evidence based.

To undertake independent assessment and management of service users to incorporate pharmacological considerations, where appropriate making complex clinical decisions regarding service user management and clinical outcomes.

To provide enhanced clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and enhanced decision making skills.

To support the senior ward team to manage change, to monitor and improve standards through supervision, evidence based practice, clinical audit, research and education. To promote and demonstrate best practice by integrating evidence into practice.
