Two exciting opportunities have arisen in our Critical Care Outreach team. The applicants must be enthusiastic, driven and focus on excellent patient care. As part of a well established service, you will be expected to continue to support wards and medical teams in the recognition and treatment of the acutely ill and deteriorating patients within our care.

We are recruiting to a 37.5 hour post. The role involves working across 7 days a week, including both day and night shifts. The role involves working at both the Royal Albert Edward Infirmary site and Wrightington Hospital site.

The post holder will co-ordinate, deliver and evaluate Critical Care within the Outreach Service. The role will facilitate the recognition and management of the acutely ill patient. The post holder will share acute and critical care expertise through educating, both formally and informally, and assisting the multi-disciplinary team in patient assessment and clinical decision-making within the ward environment in relation to this patient group.

Perform comprehensive patient assessment, based on history taking and physical
examination, suggesting solutions and making recommendations.Performs a comprehensive assessment of the patient’s needs, plan, implement and evaluates care delivery in accordance with their individual health care needs and varying level of complexity

Provides expert nursing intervention and makes complex judgements when specialist knowledge and experience are required in ensuring optimum care and treatment is maintained

Monitors and maintains the environment of care to ensure the highest standard is provided

Requests and interprets relevant investigations, e.g., chest Xrays, ECGs, blood tests with in the scope of professional practice and communicates findings to appropriate members of the multi-professional team

Identifies patient problems, and initiates appropriate nursing intervention, prescribing appropriate drug and fluid therapy utilising Patient Group Direction

To act as a self directed practitioner who directs, delivers and evaluates patient care founded on nursing and medical practice model.

Participate in the provision of direct patient care supporting the ward team in the management of unstable patients

Demonstrate critical thinking, problem solving and decision making skills

Make and receive direct referrals to and from the multi-professional teams

Responds to requests from clinical staff in ward areas for urgent/emergency review of patients at risk of clinical deterioration and provides advice and direct support to nursing and medical staff in relation to acute illness management

Provide the clinical expertise and experience to facilitate the safe transfer of critically ill
patients between clinical environments

Provide support and advice in managing patients with specific needs, e.g., Acute illness, sepsis, follow-up from ICU/HDU and implementation of various pathways, central venous catheters, tracheotomies and chest drains

Maintains personal, professional knowledge and understanding of current issues relating to

Critical Care Outreach through commitment to self-development and through continuing study.

Able to teach all disciplines of clinical staff, providing agreed teaching/education sessions in line

Understand and recognise own limitations
