We are looking for a highly experienced mental health practitioner to lead and coordinate the Hub day to day functioning and clinical servicedelivery. This include providing support and guidance to staff within the Hub. You will be working as part of an MDT and provide a lead role intriage, assessment and offering short-term evidence based interventions to patients requiring mental health support. This role needs someonewho is able to work collaboratively and build closer links between primary care networks and Secondary Care services, Social Care teams,community services and a wide range of voluntary sector organisations, to facilitate smooth transition between teams and services.
