An exciting opportunity has arisen for a Gynaecology Support Worker and patient centred follow up coordinator post to work at Ipswich Hospital.

You will be working closely with the Clinical nurse specialists (CNS) to support patients and ensure their care is personalised, You will carry out holistic needs assessments and care planning, seeing patients in clinic and the chemotherapy day unit.

You will ensure that patients’ needs are met and that they are aware of the wellbeing services and support they can access. You will ensure they are referred for prehabilitation and rehabilitation services as appropriate. You will enrol patients onto the remote surveillance system (PCFU) and be the first point of contact for patients using this follow up service. You will work with the clinical team to scope and deliver a support group for patients and sign post and refer them to all support services as appropriate.

The role will require you to be able to work independently although you will have close support with other teams involved in the patients care to promote, recruit and support uptake to wellbeing services and exercise initiatives and ensure data collection and evaluation of services is undertaken.

To work as an integral part of the gynae/oncology team as an administrator within the Patient Centred Follow Up (PCFU) service

To provide an administrative support to the Gynae Onc CNS team, wider MDT & cancer network

To provide a communication link between hospital, patients and GPs

To be a key contact for all patients on the cancer pathway

Oversee the running of the gynae cancer support group in conjunction with patients & the CNS team

Nurturing good working relationships with other support workers/PCFU coordinators especially at the cancer units
