**We are hiring a Rehabilitation & Wellbeing Worker - this role will be a big part of an exciting new initiative as part of Open Mental Health, where we value and adhere to a philosophy of ‘no wrong door’**

You will join a new Community Rehabilitation Team, which will specialise in working with people with longer-term mental health support needs. The service will offer specialist rehabilitation assessment, support, and assistance to people with more complex mental health problems.

The service’s focus will be on supporting people to develop skills and confidence to live independently and to work towards each person’s own recovery goals and improved mental well-being. The service has high aspirations of recovery and well-being for everyone it supports.

Our aim is to reach all of Somerset’s communities and to ensure we do this, we have developed strong links with a range of organisations across the county.

You will be working as part of a multi-agency team made up of team members from the Voluntary Sector Alliance and Somerset Foundation Trust.

Your role will be to work using a ‘whole-team’ approach to support individuals with long-term enduring mental health challenges to develop a well-rounded and long-lasting recovery within the community. Using a psychologically informed approach you will use a range of recovery and crisis management tools to support individuals to develop self-management skills, make informed decisions, maximise their independence and be part of their local community, accessing housing and other services that they are entitled to which benefit and interest them.

**Who am I ? - we would love to hear from you if you have the following experience**
- Knowledge of support needs of people with moderate to severe mental health needs
- Knowledge of health and safety issues, especially those relevant to mental health
- Knowledge of safeguarding policies and procedures relating to adults and children and young people
- Knowledge of equal opportunities issues
- A minimum of two years’ significant experience working with people with moderate to severe mental health needs.
- Experience of working with people who have experienced multiple disadvantages (e.g. mental health, homelessness, alcohol/drug dependency long term conditions)
- Experience of carrying out needs and/or risk assessments and the support planning process.
- Experience of time management and competing priorities when supporting people with complex needs.

**Base**

You will work across the following venues: Somerset on site in Yeovil (The Foyer), Wells (Keward House), and Bridgwater (Willow Ward). Team office space will be established in Bridgewater, and the role will include some home and community working.

**Hours**

Support will be offered 7 days a week, 10am - 8pm and you will work on an agile basis in line with an agreed rota. These are full-time roles to be worked flexibly including evenings and at the weekends.
