The Criminal Justice Care Coordinator worker role aims to improve the lives and wellbeing of people with multiple needs who require intensive support and may find it difficult to navigate or engage with services, revolve in and out of services or have barriers to accessing services.

The worker will use a trauma informed strength-based approach to assess a person and enable them to access the services they need that offer a range of evidence based interventions to help people increase their resilience, gain confidence, acquire the personal and social assets they need to meet their goals, reduce risks and gain/maintain suitable, stable accommodation.

To work as part of an integrated team providing collaborative case management to substance misusers in contact with the criminal justice system. To provide an effective link with prison and court establishments, supporting service users entering and exiting prison custody. To provide holistic and structured recovery coordination and psychosocial interventions to service clients, their families and carers. Working to ensure effective delivery of recovery coordination within the full integrated service model.

The worker will develop a comprehensive data base that will track service users from court appearance, their journey while in custody and their discharge. The worker will assess and develop care plans for the person’s release while in custody. This will include links and referrals to housing, treatment services and continuity of medication and CMHTs. The person swill attend courts that cover Powys and seek ways to maintain a presence and contact with offenders who will appear at Merthyr Courts

**Job Types**: Full-time, Fixed term contract
Contract length: 9 months

Pay: £22,500.00-£24,500.00 per year
