DCHS have an exciting opportunity for a Nursing Associate to join the Derby City District Nursing Team.

Integral to this role is the ability to communicate effectively, with sensitivity and compassion and to provide nursing expertise with patients in our community. Our District Nurses provide extensive support and nursing care to patients in Derby.

Repton Village District Nursing Team is a learning team who share experiences to improve patient care in a balanced and supportive way. We provide an array of services to our local community including many different types of wound care, end of life care, rehabilitation, disease/ medication management to support patients remain in their home.

In addition to this, we work very closely with our therapy colleagues and local GP surgeries, providing a varied and interesting role in the community.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

- Provide compassionate, safe and effective care and support to patients and service users in a range of care settings;
- Monitor the condition and health needs of people within their care on a continual basis in partnership with patients, families and carers and other health professionals.
- Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment within the boundaries of confidentiality;
- Communicate effectively and appropriately with colleagues, providing clear verbal, non-verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility of care;
- Recognise and respond appropriately to a patient/ service user whose condition is deteriorating.
- Work within own competence.
- Improve the quality of care by contributing to the continuous monitoring of patient’s/service users experience of care;
- Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of patient’s/service user first.
- Contribute to the provision of care for patients/service user, including those with complex needs;
- Understand the roles of a range of professionals and carer from other organisations and settings who may be participating in the care of a patient/ service user and their family;
- Understand responsibilities in relation to communication and collaboration.
